CTIntake Premium

With CTIntake Premium you can eliminate time-consuming data entry by importing patient responses from your CT Intake form into your patient chart notes. Use CTIntake Premium in conjunction with your own custom macros or our lightning fast BulletTouch macros to create your own bullet-proof documentation and automate your new patient intake and new patient narratives. Patients can fill out their intake forms via the web at their convenience, or via iPad as they sit in your waiting room.

Importing Intake Information with BulletTouch Macros

To sign up for CTIntake Premium:

  1. Log into www.mychirotouch.com.
  2. Click the yellow Services button.
  3. Click CTIntake Premium.
  4. Enter your payment information to sign up for the service.

You can configure your CTIntake Premium via MyChiroTouch.com

  1. Log in to www.mychirotouch.com.
  2. Click the yellow Services button.
  3. Click CTIntake Premium. Once you have signed up for this service, you can use this page to configure your patient intake form.

Closed Customize Your Patient Intake

In the Customize Your Patient Intake section, select your categories. There are two default and six optional categories. Default categories include: About You, and Reason for Visit. Select the optional question categories to add these sections to your patient intake form.

IMPORTANT:  These options are not automatically configured. You must select the appropriate categories for your particular practice and patients.

The sections For Women Only and For Men Only will only be displayed for patients over the age of 16.

ClosedAdd a Spanish Language Option, if Needed

By adding a check to this box you enable a Spanish language option in the intake form:

When selected, additional Spanish-related options appear:

  • Authorization Message
  • Welcome Message
  • Completed-form output

What the Patient Will See

When enabled, the patients will see a "Language Preference" choice at the top of the Intake Form. By selecting "Español", all questions, prompts, and notices will be presented in Spanish.

As the choice appears in the Intake Form: 

ClosedSelect an Authorization Message

  • Select Standard to use the authorization message that appears in the text box. (See Spanish language option above.)
  • Select Custom to edit the authorization message in the text box.

Be aware that these are affected when adding the Spanish language option above.

NOTE:  The authorization message set here applies to both the web version and CTIntake iPad app.

ClosedChoose your Web Only Settings

Select a logo image and choose from standard or custom contact information.

NOTE:  These settings are only displayed on the web and will not display on the CTIntake Premium iPad app.

ClosedSelect Your Welcome Message and Sign-In Requirements

  • Welcome Message – Select to use either the Standard welcome message, or select the "Custom" radio button and enter in a custom welcome message. Be aware that this is affected when adding the Spanish language option above.
  • Captcha – If you would like to require a CAPTCHA to protect against potential spam, select the "Yes" radio button.
  • Patient Signature – To require a patient signature upon sign-in, select the "Yes" radio button.
  • Preview – Click the Preview button to preview your web patient intake form.
  • Save – Save your patient intake form by clicking the "Save" button.

Closed(Optional) Generate a Printable Version of Your Patient Intake Form

Once you have configured your Patient Intake form, you may generate a printable version of this form for patients who experience difficulties with technologically-presented materials or would otherwise prefer a printed version of your patient intake form. This printable patient intake form reflects any customizations you have selected.

If you opt to include a Personal Injury intake form into your patient intake process, a printable "Supplemental Intake Form (Personal Injury)" PDF link will be available here as well.

You have the option to collect patient intake forms either at your patients' convenience via the web, or while they await their appointment in your waiting room.

ClosedTo Collect Patient Intake Forms Via the Web

Provide your patients with your ChiroTouch Patient Intake Web page by e-mailling them directly or incorporating your custom web intake page into your website.

ClosedTo Collect Patient Intake Forms Via iPad

To enable CT Intake Premium on the iPad(s) in your practice:

  1. Download CT Intake Mobile from the iTunes app store.
  2. Log in to the CT Intake Mobile application.
  3. Tap the settings icon at the bottom of the screen.
  4. In the Settings screen, tap Form Customization in the sidebar.
  5. Tap this option to enable the Documentation Macro-enabled Form.
  6. Tap Save.


The patient's iPad intake form will populate the macro-enabled question categories you selected during your Intake form setup process. Based on the patient's Case Type, the particular questions posed to your patient will differ.

Turn the iPad Intake Form over to Patients for Completion

  1. Log in to the CT Intake Premium application and enable settings for CT Intake Premium.
  2. Tap each text field to enter the patient's information.
  3. Tap to select a value for the Case Type field.
  4. Tap Next.

The CTIntake/CTMIntake Premium application provides patients with easy-to navigate pages and streamlined information capture.

Minimal, specifically tailored questions require a short amount of attention from the patient. On the iPad, patient-facing screens can be rotated into portrait format for better usability.

ClosedAbout You Page

The About You section contains generic demographic questions for the patient to fill out, including address, date of birth, referral information, and height and weight. On this page patients will find:

  • Drop-down selections reduce the patient burden of data entry
  • A separate section with height and weight scales defaulted to the lowest available value in an effort to help patients clearly see that these values need to be adjusted
  • As the patient tabs from one question to the next, the page will scroll automatically
  • To match the standard experience when using an iOS device, patients using the CTM Intake Premium application will experience auto-capitalization of names for an easier, more familiar experience
  • Fields update in response to gender selection to reflect gender-specific questions and answers

If your patient neglects to complete any required fields, a red outline will appear around the required field(s) after they click Submit, and an Alert message will appear stating: "Please complete the required fields in red and then tap Next".

NOTE:  At least one phone number is required when completing the patient intake form.

ClosedReason for Visit Page

The Reason for Visit section is a dynamic page that adjusts to the patient's responses. Content on the "Reason for Visit" page dynamically adjusts when one of the five reasons available is selected from the first drop-down. Once the patient selects their reason, all page content will adjust to address the type of visit selected.

On this page patients will find:

  • The option to add up to four Reasons for Visit
  • Upon returning to edit a Reason for Visit, each page will display a unique title to help the patient understand which Reason for Visit they are editing
  • A severity scale with clear severity indicators to help patients answer the question in a meaningful fashion.
  • Labeled front and back body indicators for easy selection of body parts

ClosedHealth Information Page

The Health Information page allows patients to include information about health history and additional health concerns.

ClosedSummary Page

The Summary Page is the last screen in the Patient Intake experience. This page provides the patient with a high level summary of the information they have entered. On this page the patient will find:

  • Edit buttons associated with each section so that patients may make corrections to their information.
  • An Acknowledgement/Informed Consent to Treatment section with signature section
  • Save and Submit button to submit their completed patient intake form.

Upon success, patients will be presented with the following message:

To import the data patient data from their CT Intake form, you must first create a new patient or select an existing patient from the Front Desk>Patient Mgmt screen.

ClosedCreate a new patient

  1. In the CT Front Desk application, click the Patient Management button.
  2. Click the New Patient button.
  3. Enter the patient's information.
  4. Click the OK button. The Patient Information screen appears.

ClosedImport the data from the form

  1. On the Patient Information screen, click the Import button. The Patient Intake dialog box appears.
  2. Select the Intake Premium tab.
  3. Click the Download Latest Files button. The Patient Intake File Retrieval dialog box appears.
  4. Click Get Files to start the download. All files that have not been previously downloaded will be downloaded.
  5. Select the patient's file. A preview of the patient's information appears on the right.
  6. Click the Next button. The information from the CT Intake form appears on the left, while the current patient information appears on the right. By default, the Import New Patient Information box is selected.
    7. NOTE:   Importing the new CT Intake data will overwrite the current patient information. Deselect this option if you do not want to overwrite existing data.

  8. The Build Forms section appears with a list of your custom intake forms with the most recently used form preselected, as well as any other forms you are using with the CT Intake iPad app.
  9. Click the Finish button. The form will be stored in the patient's records directory, and the Import Complete dialog box appears.
  10. Click the OK button to return to the Patient Information screen. The new patient data appears in the fields.

ClosedView or print a patient's PDF form

On the Patient Information screen, click the Records button. The CT Intake PDF forms appear on the right. Double-click the file to open it.

To review patient intake history, click the Patient History tab. This screen allows you to view records of patients who have completed their patient intake forms. Select to view "Downloaded" or "Not Downloaded" records. Click the column headers to sort by Name, Email, and Created date.
