Documentation of Current Medications in the Medical Record (CMS-68)
Description
This measure requires that a study be conducted for a percentage of visits for patients aged 18 years and older for which the eligible professional attests to documenting a list of current medications using all immediate resources available on the date of the encounter. This list must include ALL known prescriptions, over-the-counters, herbals, and vitamin/mineral/dietary (nutritional) supplements AND must contain the medications' name, dosage, frequency and route of administration.
Eligible Patients (Denominator)
The following patient eligibility criteria along with a Current Procedural Terminology (CPT) encounter code set mentioned below include your patient into the Denominator for this measure:
- Patients 18 years of age or older before the start of the measurement period, and
- Had an office visit (see below) during the measurement period
The qualifying office visit must be among the Medications Encounter Code Set.

Completing Compliance (Numerator)
For patients aged 18 years or older who have had a least one visit within the measure period, record their current prescribed and over-the-counter medication information via the QPP Checkout Wizard or through the Patients application. If medication had already been added, review and verify that the information is current.
Record active medication through the Front Desk Checkout Wizard:
- Select a checked-in patient from your Front Desk and click Checkout.
- On the Check Out screen select the QPP Wizard.
- Select Click Here to launch CTRx at the top of the Objective screen to enter the CTRx service. (Shown below)
- Click Yes to the prompt verifying the appointment date. Enter Patient medication by clicking Manage Medications at the top of the CTRx application.
Or through the Patients application:
- Select the Patients application from your Launcher (if this is not currently present on your Launcher it can be added through Options by right-clicking the hand icon to the top).
- Search for your patient and double-click once the appropriate patient account is visible.
- Click the Appointments tab, and Past just below to select the correct appointment date to apply your Quality information.
- Click Launch Patient CTRx Service under the Actions header to the left.
- Click Yes to the prompt verifying the appointment date. Enter Patient medication by clicking Manage Medications at the top of the CTRx application.
