Input Options
There are three options for gathering your patients' data:
ChiroMatrix is an easy way to set up a web page and integrate CTIntake. With ChiroMatrix, the work is done for you. ChiroMatrix will set up your practice website and integrate their iForm web intake page for you.

Create a custom web intake page through MyChiroTouch. Then place a URL to the intake page on your existing website, or simply provide the URL to your patients via e-mail or otherwise.
You create your own web page and pass that information to the ChiroTouch web server. We highly suggest that you do not use your own form unless you are a skilled web professional.
